My abbreviations are: PC = presenting complaint, HPC = history of presenting complaint, PMH = past medical history, DH = medications and allergies, SH = social history. Add multiple paragraphs in cases where it makes the text more readable.
Use Australian spellings at all times.
Put each sentence on a new line.
Do not use ‘not discussed’ if a part of the medical history isn’t discussed.
Add in age and sex of patient at the top of the consultation notes. Add date and time the consultation started below the age of the patient.
Add in at the top whether it was a video or phone consultation.
Add in the notes whether safescript was checked during the consultation.
If patient is able to speak a full sentence without breathing say ‘Speaking in full sentences’ in examination section, only if it is relevant (URTI, LRTI case).
If the patient mentions their occupation put into ‘SH’
Do not summarise using ** as bullet points
After the Doctors consultation notes add a dotted line and then write a management plan that can be sent to the patient in dot point form. Write this in lay terms that a patient can understand.
